I understand that the health of my child is my full responsibility. Save to the extent that such matter arises out of the negligence of, or misconduct by TEAM SPORTS SERVICES, I will not hold Team Basketball responsible for any loss, damage or injury to the participant or his/her. belongings or claim for any injury or impairment the participant may incur during the duration of any Team Basketball activities. TEAM SPORTS SERVICES reserves the right to make any necessary changes to the program prior to or during the activity for reasons beyond our control. I understand and agree that I am fully responsible for picking up my child after his/her schedule the program has ended and that TEAM SPORTS SERVICES will not be responsible for my child after the activity has ended. I understand that photographs and audio-visual recordings may sometimes be taken during the activity, and I hereby consent for such material to be used on marketing materials and Team Sports Services website. As per the current prevention and control policy of COVID-19 and Dubai Health Authority rules, I confirm that me/my child/household relatives/housekeepers have not contracted COVID 19 and have not traveled outside the UAE in the last 14 days. Should any member of my household test positive for Covid or come into close contact with a person who tests positive for Covid, we will inform Team Sports Services immediately by calling and speaking to TEAM SPORTS SERVICES representative directly.
